The Strengthened Standards move the emphasis from process indicators to outcomes for older people. For providers, that changes the kind of evidence assessors want to see, and how you keep it.

From indicators to outcomes

Where the old model often rewarded having a policy in place, the strengthened model asks what actually changed for the people in your care. Evidence needs to show practice, not just intent.

  • Keep records that demonstrate an outcome was met, not only that a process exists.
  • Link documents to the specific outcomes they support.
  • Review evidence regularly so it reflects current practice.

Practical takeaways

Build an evidence habit: attach as you go, describe what each document demonstrates, and rate conformance honestly. A living evidence base is far easier to defend at audit than a last-minute scramble.
